    A Medical Scribe is a healthcare professional that performs patient documentation and clerical tasks on behalf of a physician. Medical scribe increases the efficiency and productivity of physicians by completing the necessary documentation and clerical duties related to patient care.

Responsibilities of a Medical scribe includes:

• Preparing referral letters for the physician or a summary of the patient's medical records.
• Coordinating referrals, preparing reports, manning the telephones and performing other clerical tasks.

• While completing a chart he must be able to accurately write the symptoms, lab findings, values and tests.

                                                     A  medical scribe allows a physician to directly interact with a patient free from the distraction of forms and notes.
